Registration and Forms Checklist

When you decide to be part of Alpha-Bet Preschool family, here is the list of forms you will need to complete and when, but don't worry, all the required forms will be emailed to you or you'll be able to access them through our Family Portal, once your account has been established.

Due at time of Registration

 

Due no later than the first Monday in August

  • Per Student:

    • Child’s Birth Certificate (original - to be seen by a school official)

    • Last full month’s tuition (May)**

    • Authorization to Release Your Child

    • Emergency Information Form

    • Student Emergency Medical Authorization

    • Media Permission Form

    • Confidentiality Statement

    • Release of Liability Waiver

    • Cooperative Membership Agreement

    • Policies and Procedures Manual Receipt Form

 

  • Per each Co-op Volunteer:

    • Driver’s License (photocopy to be kept in file)

    • VA Department of Social Services “Sworn Statement of Affirmation”

    • VA Department of Social Services “Central Registry Release of Information Form”

    • Fieldprint Fingerprinting (fee of $59 must be paid online when scheduling your appointment at www.FieldprintVirginia.com)

    • Report of TB Screening (physician’s signed document may be substituted)

    • Two written references (from non-family members) stating how they know you, how long they’ve known you, and that you are of sound character and judgement, and capable of being responsible for young children.

    • Dates in September that you are available to assist in the classroom (siblings are not allowed to be in the classroom on co-op days) 

 

Due before First Day of School

  • 1st Month’s Tuition**

  • Supply/Administrative fee**
    (May be paid in full or half of amount, with remainder due January 1st)

    • 3 Day Program: $125

    • 5 Day Program: $150

  • VA School Health Form (including updated immunizations) completed and signed by physician*
