Due at time of Registration

  • Registration Form (

  • Child’s Birth Certificate (Photocopy to be kept on file)

  • Registration fee  (While normally nonrefundable, in the event that we are unable to open due to health concerns, this fee will be refunded)

    • Early Bird Registration (through April 1st) - $100​

    • Registration after April 1st - $150

Due by August 1, 2021 (any forms will be provided once we have processed your registration)

Per Student:

  • Last full month’s tuition (May 2022)

  • Registration Packet

    • Authorization to Release Your Child

    • Emergency Information Form

    • Student Emergency Medical Authorization

    • Media Permission Form

    • Confidentiality Statement

    • and Cooperative Membership Agreement)​

  • VA School Health Form  (including updated immunizations) completed & signed by Physician

Per each Co-op Volunteer:

  • Driver’s License (Photocopy to be kept on file)

    • Report of TB Screening* (Physician’s signed document may be substituted)

    • VA Department of Social Services/Child Protective Services Central Registry Release of Information Form*

    • VA Department of Social Services “Sworn Statement of Affirmation”*

    • VA State Police Combined Criminal History & Sex Offender Information & Consent*

    • Approved Fingerprint through Field Print
